Spouse Contributor

Spouse Contributor is intended to assist the Participant with future contributions to the TakafulLink plan in the event of death, TPD before the age of 60, or diagnosis of a critical illness on the spouse of Participant. Payment of annual benefit will cease upon death of the Participant or upon reaching age 60.

Major Benefits Benefits Benefit is payable if the spouse of the Participant is diagnosed to be suffering from a Critical Illness; or has become totally and permanently disabled prior to attaining age 60; or has died Limitations Only one claim under this Benefit can be made regardless of the number of Critical Illnesses diagnosed, except for full-blown AIDS or if TPD occurs with Critical Illness

Indicative Contribution Rates Annual contribution per RM100 contribution for Participant whose spouse is aged 35. Age of person covered: Spouse Contributor Aged 35 RM21.67 Aged 45 RM14.78

Important terms and conditions of the product The Takaful Operator reserves the right to increase the contribution and Tabarru' at any time by giving a 90-day written notice to the Participant at the last known address. Contribution and Tabarru' revision shall only be effective at the next certificate anniversary date. Should the next certificate anniversary date fall within 90 days from the date of such notice, contribution and Tabarru' revision shall then be effective at the following certificate anniversary date. You are to disclose in this proposal form (and in any personal statement made to the Takaful Operator or to the Medical Examiner of the Takaful Operator) fully and faithfully, all the facts which you know and ought to know, otherwise the certificate issued hereunder may be invalidated. Exclusions The benefits shall not be payable: 3.1 If the symptoms of the Critical Illness is manifested prior to or; Within sixty (60) days for Heart Attack, Coronary Artery Heart Disease requiring Surgery, Cancer or other Serious Coronary Artery Heart Disease of; or Within thirty (30) days for all other Critical Illness listed in the Glossary of Terms of The date of this Certificate; or The date of issue of this Annexure; or The date of Revival; or The date of Endorsement; or The commencement date of the Benefit; whichever is the latest. 3.2 If the participant is diagnosed to have a Critical Illness that is caused directly or indirectly by the existence of Acquired Immune Deficiency Syndrome (AIDS) or the presence of any Human Immunodeficiency Virus infection. The exception is when AIDS is full-blown or is caused by blood transfusion.
